If you would like to join our practice, please call our new patient coordinator to find out which of our doctors and nurse practitioners are open to new patients. Because your insurance company website may not be up to date, calling our office is the best way to know which doctors are accepting new patients.
If you are expecting a new baby or adopting, we encourage you to schedule a prenatal appointment to meet us, see our office, get information about our practice and ask any questions you may have.
The first time you visit our office, there will be a lot of information to gather, including records from your past doctors, immunization records, contact information, insurance information and past medical history. In addition, we need to give you information about our privacy practices, financial policies and have you sign to confirm that you have received that information. To makes things go smoother at that first visit, you can fill out the forms ahead of time on your computer, print them and either mail them to us or bring them with you.

Patient Registration Form: All patients need to fill out this form. Please fill it out as completely as possible, as all of this information is important for us.
Medical and Family History: Needs to be filled out for all new patients. For new babies (and children under 1 year old), please fill out the alternative New Baby Questionnaire.
Please fill these out as completely as possible, to help us learn about your child’s past medical history and elements of your family history that may affect your child’s health.
Release of Medical Records: This form allows us to get records from your previous doctors. You can send this to them yourself or bring it to the visit and we will fax it for you. This form is not necessary for new babies.
Financial Policy: Is for you to read and keep. Please read these and ask us if you have any questions. This is our office policy related to how we bill you and your insurance for services that are provided.
Notice of Privacy Practices: Is for you to read and keep. Please read these and ask us if you have any questions. These are our office policies related to the privacy of your child’s medical records.
Acknowledgement of Receipt of Privacy Practices: Shows that you received the Notice of Privacy Practices. All patients need to sign this form and turn it in to us.

You can call either office to schedule an appointment for your child. We book appointments up to three months in advance. You are encouraged to schedule well visits as early as possible so that you have the most choice of days and times for your appointment.
We are committed to seeing your child the same day if you feel they should be seen.
During our office hours, the advice nurse is available to answer any questions you might have. If your child needs to be seen, a same day appointment will be scheduled.

Flu vaccine is recommended for children 6 months to 18 years of age during the Flu season and is highly recommended for high risk children (children with asthma, respiratory problems, chronic disease) and siblings and parents of infants less than 6 months of age.

We are happy to be able to provide new mothers and babies with excellent lactation support in our office, provided by Brenda Evans, PNP, CLC. Appointments for lactation consultations can be scheduled in advance, and usually are available as a same day appointment as well. In addition, we have some lactation supplies for sale, such as nipple shields, supplemental feeding systems and pacifiers. Brenda is also available to answer non-urgent breastfeeding questions by calling the “Milk Line” at (415) 459-MILK (6455).

In addition to the Advice Nurse, After Hours Clinic and Emergency Room, there is always a physician on call for our practice. Our answering service and advice nurse will call the physician if necessary after hours.

You should contact your insurer directly about what your specific insurance policy does and does not cover. If you have any questions about whether a visit or service will be covered, please call your insurance company before the visit. We are happy to give you any information that will help you do this, and can share the estimated charges with you ahead of time.
